Caring in the Community
Our dedicated medical professionals provide a range of health care and support services out in our communities across Brent, Harrow and Ealing including sexual health, end-of-life care and physiotherapy for children. In addition, we have two outstanding rehabilitation centres at Clayponds and Willesden, and Meadow House Hospice which provides palliative care to patients with life-limiting conditions.
Rehabilitation centres of excellence
Our rehabilitation centres aim to enable adult patients to return home and live as independently as possible following a period of medical, nursing and therapy care. Specialising in neurological conditions, neuro-rehabilitation for stroke, brain injury and other conditions, as well as general rehabilitative care, the centres also provide active outpatient therapy teams treating a range of conditions including musculo-skeletal conditions, speech and language therapy as well as psychological support.
Serving the population of the boroughs of Ealing and Hounslow, Meadow House cares for people who have a life limiting or terminal condition, providing support to their families and carers as well. The specialist team is dedicated to show compassion, care, respect and dignity at all times to people whose illnesses are no longer curable, helping them to achieve the best possible quality of life during the final stages of their illness.
District nursing teams
The District Nursing service provides nursing care to patients over 16 years of age who are housebound due to a short or long-term condition and who would otherwise find it difficult to access regular healthcare. The District Nurses are closely linked with local GP services and work in partnership with other health and social care professionals to provide a fully integrated service to patients throughout North West London.
You can be referred to the District Nursing service by your GP, Practice Nurse, a hospital or other health and social care professionals.
